Skills for Independent Living (SkILs) reablement

The Skills for Independent Living (SkILs) reablement service supports adults to look after themselves independently, following a change in circumstance, illness, injury or time spent in hospital.

Reablement forms part of Active Recovery, an integrated health and social care service to help people regain independence after illness, injury, or a hospital stay. 

Active Recovery brings together health professionals and social care teams to provide short-term rehabilitation and reablement support at home, focusing on what matters most to each person. 

By combining therapy and practical assistance in one integrated service, we ensure a smooth, coordinated approach that avoids duplication and helps people recover faster, stay well, and live confidently in their own homes.

Who we help

Reablement is different to traditional home care. We help and support people to:

  • regain and develop the confidence and skills they need to stay living in their own home for as long as they can
  • do as much as possible for themselves rather than having others do things for them

What we do

Support we offer includes:

  • a short-term service delivered in a person’s own home which supports them to maximise their independence, improve their quality of life and achieve their desired outcomes
  • help for people to regain their ability to do everyday tasks or use equipment to maintain these abilities

We work closely with health partners and the third sector to facilitate admissions and ensure smooth and timely discharge from hospital to home.
