LCC adult day opportunities

We provide support for adults living with dementia, memory impairment, mental health, physical impairment or complex care needs.

We work collaboratively with Adults Health and Social Care, communities and partners (including the community mental health teams), acute services, specialist services, primary care, the voluntary sector and third sector organisations.

What we do

We have 4 centres that run city-wide community-based day programs.

The core features of our day opportunities are:

  • community integration: helping people take part in everyday local life rather than staying isolated
  • skill building: supporting adults to learn independent living tasks like cooking, budgeting, or using public transport
  • health and wellbeing: access to gym facilities, leisure and recreational opportunities and support to increase exercise, stop smoking, lose weight and more
  • peer support: creating friendship groups and support networks both in the centres and in their local communities
  • meaningful activities: offering paths for volunteering, work experience, education, arts, sports, and social clubs
  • social connection: providing a welcoming place to make friends and reduce loneliness
  • carer support: giving family members and regular caregivers a reliable break or respite time

The support we provide includes group activities and one-to-one support, including support with personal care, as well as information and signposting, carers support and peer support groups.

All centres have changing places to support with personal care and moving and handling.

A Saturday service for complex needs customers also runs at our Laurel Bank centre.

How to get support or make a referral

To make a referral for any of our day opportunity services, contact our centres directly.

We aim to process referrals within 14 working days. You will be contacted by one of our assessors to arrange an initial meeting to discuss the service and assess your suitability.

All services (except mental health day opportunities) are chargeable. Find further information on potential charges.

All charges are means tested and we will work with you to work out how much you can afford to pay towards your care. Find more information on getting a financial assessment.

For mental health support day opportunities, one-to-one support may be chargeable dependent on the level of support needed. This will be discussed with you by the service when planning your care.

Our physical impairment service located at Holt Park Active is a citywide service for adults with physical impairments and older people.

Early support service

LCC day opportunities also offer a free, 8-week early support service for people living with dementia.

The key features of the service are:

  • no social worker needed: you can access the free 8-week trial by calling directly, provided you meet the criteria.
  • care and support: providing the opportunity to have immediate access to the service, and all elements of the service activities, lessen social isolation, as well as develop peer support and companionship

Individuals wanting to access the service must be able to access the service independently, or with support from family or friends.

We cannot support people with medication or personal assistance needs during the 8 weeks.

Mental health day opportunities
