If you want to authorise the sale or supply of alcohol on licensed premises, or if you apply for a lot of temporary event notices you might want to apply for a personal licence.

Change your name or address on your personal licenceChange your name or address on your personal licence<div class="ExternalClass05DEDE9103AC4ED99AFC10AFC97C52DD">​​<p>If you would like to change the name or address on your personal licence you can write to us telling us what you would like changed. If you would like to change your name, you should include a proof of your change of name, i.e. your marriage certificate. Apply for a new licenceApply for a new licence<div class="ExternalClass77D6EA34E9D6475097C11E25C10CF0F9"><p>If you would like to apply for a new personal licence you will need to complete the application forms <strong><a href="/docs/PERS%201%20-%20Personal%20Licence.doc">PERS 1 (Word doc 124KB)</a></strong> and <strong><a href="/docs/PERS%202%20-%20Disclosure%20of%20convictions%20and%20declaration.doc">PERS 2 (Word doc 90KB)</a></strong>. We also have <strong><a href="/docs/GN-O%20LA2003%20New%20personal%20licence.pdf">some guidance (PDF 308KB)</a></strong> on how to fill in the application. This includes two passport sized photographs, proof of your right to work in the UK, a DBS basic disclosure and a personal licence qualification. It will cost £37 and you should send your application and supporting documents to the address on the form.</p> <p>Because you have to provide original documents you are not able to apply for this licence online, or by email but you can post us your application and pay by cheque or we can take payment over the phone on <strong>0113 378 5029</strong>.</p> <p>There are set timescales stated in the legislation for determining a personal licence but we will advise you when it has been granted. Usually personal licence applications are straightforward and are processed quickly. Tell us if you have been convicted of a relevant offenceTell us if you have been convicted of a relevant offence<div class="ExternalClassB476DED576A94608BF2996531BF23218">​​<p>If you have been convicted of a relevant offence you have a responsibility to tell us so that we can endorse the paper part of your personal licence. Only the court has the ability to revoke your licence and so if they haven’t asked for your licence at the court hearing, you should contact us as soon as possible. Lost licencesLost licences<div class="ExternalClass46CB3908234A485491A99652EF2AB42B"><p>If you have lost your personal licence, please contact the Entertainment Licensing team at <a href="mailto:entertainment.licensing@leeds.gov.uk"><strong>entertainment.licensing@leeds.gov.uk</strong></a> to request a replacement. The fee for a replacement licence is £10.50 - please provide your contact number within the email so that an officer can call you back to take the payment securely over the phone.</p></div>
