Leeds Transition Plan (converting statements of SEN and Learning Difficulty Assessments into EHC plans)

The Leeds Transition Plan gives an overview of the transitional arrangements made by Leeds City Council in transferring statements of SEN and Learning Difficulty Assessments (LDAs), into new Education, Health and Care Plans. 

The Leeds Transition Plan is made up of a number of different documents as set out below, all of which are available below:

  • Transition plan overview

  • Conversions guidance for families and young people
  • This gives an overview of the conversion process and statutory timescales involved in conversions of both Statements and LDAs. It also gives some detail about what you can expect to happen in the conversion meeting.

  • Conversions guidance for schools, settings and professionals
  • This gives an overview to professionals, schools and other settings about the conversion process and statutory timescales of both Statements and LDAs. It also gives details about how to conduct the conversion meeting, and some further information about how to complete the conversion meeting proforma.

  • Conversion Schedule
  • This sets out the Local Authority’s plan of how all the existing statements will be converted into new EHCs over the three year period. There are two slightly different plans for converting statements – one for children/young people in mainstream and resource provision settings; and one for children/young people in SILCs (Specialist Inclusive Learning Centres) and SILC partnership settings.

  • LDA Conversion Schedule
  • This sets out the Local Authority’s plan on how LDA conversions will be prioritised.

  • Transition Plan Progress Report 2016 - Infographic
  • An infographic which sets out how many conversions have been completed and what the next priorities are in an accessible, easy-to-read format.

    Please note that currently, due to increasing pressures on capacity, there is a small backlog of conversions where parents/young people are waiting for the local authority to complete a draft EHC plan. If you have any concerns about the timeliness of your/your child’s draft EHC plan following conversion please contact the SENSAP team by emailing sensap@leeds.gov.uk and we will do our best to inform you of our progress.

    Parents and young people can seek advice and support regarding their conversion from a statement or LDA to an EHC Plan from Leeds SEND Information Advice Support Service who will be able to offer support on any of the above services.
