Who we help

Spring Gardens

We support people who:

  • are older and require 24-hour care and support
  • may have a diagnosis of dementia
  • may have mental health support needs
  • have physical needs which may include support with personal care and mobility

Dolphin Manor

We support people who require short stay dementia care.

Dolphin Manor provides an innovative, multidisciplinary service for people living with dementia, offering a range of short stay services including:

  • transfer from hospital
  • urgent response to crisis or alternative to hospital admission
  • reduction of one-to-one staffing
  • planned short breaks (respite care)

What we do

Support we offer includes:

  • help with all aspects of personal care, health and wellbeing in a safe and comfortable home environment
  • strength based approaches to support people to retain their independence and do what they enjoy with the people who are important to them
  • supported individual and group activities at home and in the community

Dolphin Manor

Dolphin Manor is operated as two units, The Oaks and The Elms.

The Oaks

The Oaks supports people who:

  • need more activity and occupation to remain calm and settled
  • require a higher staffing level to ensure safe care

The Elms

The Elms supports people who:

  • require skilled staff to provide person-centred care

Generally, The Elms accommodates people with mild to moderate dementia and lower levels of need and risk compared to The Oaks.

Working with NHS partners, the service supports people with a level of psychological and behavioural needs, to develop the right person-centred care and recover well-being. For some people, the service will enable a return home - for others, the outcome will be choices for less specialised, less restrictive care.

Respite care

Dolphin Manor has three beds available for people living with dementia to book planned short stays in advance.

How to get support

A formal referral will usually come from the social worker, or other professional role responsible for co-ordinating community care and hospital discharge.
