The Transitions Team works with young people aged 14-18 who have complex impairments and/or health needs. Referrals are taken from age 14 to age 16.
We offer an inter-agency approach to assisting young people and their families in co-ordinating the move from childhood to adulthood.
We work closely with schools, the Careers Service and other agencies to identify appropriate resources for young people. We also offer information, advice and support to young people, families, carers and other professionals regarding the transitions process.
The Team operates on a consultancy/joint working basis linking in to cases held by local authority social workers in order to assess, advise and assist.
