Social care is not about getting 'one-size-fits-all' services. People need to be able to exercise choice, and tailor their support to their individual circumstances.
When you have your self-directed assessment, you will be asked to think about the 'outcomes' you want. But what does this mean?
An 'outcome' just means something you want to achieve - how you want your life to be - not just in terms of what social care services you receive, but in a wider sense.
For example:
- How would you like your life to be?
- Who and what is important to you?
- What would you change in your life?
- What would you keep the same?
When you put together your support plan, think about how you want your life to be, and the different ways you could achieve these outcomes. It could mean using services and support from the council, other providers, access to information and services in your local community - or a mixture of all these things!
