Health and Social Care Transformation Programme

The Leeds Health and Social Care Transformation Programme is a city-wide agreement between health and social care to work together to improve people’s experience of our services and ensure we’re making best use of resources.

The programme is likely to have far-reaching effects on how services are delivered in the future, and we’ll be communicating regularly about any changes it’s set to bring – so watch this space!

The organisations involved are:

  • NHS Leeds
  • Leeds Community Healthcare
  • Leeds City Council
  • Leeds Partnerships NHS Foundation Trust
  • Leeds Teaching Hospitals NHS Trust
  • GP commissioning consortia.

Seven areas are being looked at as part of the Programme:

  • Services for older people and people with long-term conditions
  • Urgent and emergency care
  • Clinical values
  • Tertiary services
  • Children
  • Estates
  • Technology

Transforming services for older people and people with long-term conditions 

We are looking at developing a consistent, city-wide approach to identifying people who are most at risk of admission to hospital. This will help ensure that people get the right support or services at an earlier stage, so they are:

  • less likely to experience a worsening of their condition
  • less likely to need to go into hospital
  • better able to manage their condition and live as independently as possible.

We will be working more closely with health colleagues in this area, and some services may eventually be integrated, where this would reduce duplications and gaps in people's care.

Other plans 
There are plans to improve the way that people with type 2 diabetes get support and treatment, so people can be supported closer to home in non-hospital settings where possible.

And there will be more advice available to people about how diet and lifestyle changes can help them better manage their condition.

Home oxygen services are also being reviewed to ensure patients who need long-term oxygen therapy receive the right amount for their needs.

Further proposals look at reducing the numbers of people attending Accident and Emergency departments where they can be better treated elsewhere, and looking at how to improve people’s ‘pathways’ through emergency care.

All major changes will be subject to consultation with citizens, staff and partner organisations.
