If you are an adult with a physical or sensory impairment, DOTTSS can assist you to find and use a wide range of facilities in your local community.
DOTTSS can support you to:
- access facilities such as leisure centres, libraries, post offices, banks etc
- find and join support and community groups
- participate in volunteer training and voluntary work placements
- find a wide range of information about local facilities
- identify and participate in college courses and other training opportunities
- gain the confidence to use local and city centre shops.
The DOTTSS team members will meet with you to discuss your interests and the kinds of support you may require. Individual plans are tailored around your specific requirements.
We will research relevant information for you, such as building accessibility and public transport options, and provide extra support such as liaising with leisure centre staff to help you access facilities or sports groups.
A team member will then work with you on a regular basis until you feel confident enough to use the facilities yourself.
Referral guidelines
Referrals to the service can be made by an individual or through other involved professionals, e.g. social workers, OTs and day service workers.
Read through the information in the other pages on this site, eligibility criteria, service agreement etc in order to decide if outreach support may be appropriate for you.
NOTE: If not, DOTTSS may still be able to assist you. Please call for more information.
- Complete the Initial Referral form as fully as possible. This enables DOTTSS to explore preliminary options before a consultation takes place. Please post (do not fax) the completed form to DOTTSS.
NOTE: Please be as specific as possible regarding personal assistance, i.e. your personal care needs, mobility etc., as this can have an impact on the service we provide. Alternatives such as Direct Payments will be explored, as DOTTSS will be working towards gradually withdrawing support as your independence increases.
- An initial consultation will be arranged in order to discuss the application in greater detail and an Initial Consultation form will be completed.
- Risk assessments will be completed and a decision to proceed will be based on those risks.
NOTE: If you are not considered eligible for outreach support at this point, we will help you find other alternative types of support.
- A Service Agreement form will be signed and access to outreach support will begin.
- Regular reviews will be scheduled In order to monitor and evaluate use and suitability of the service for the individual.
- When appropriate, DOTTSS will begin to withdraw support as independence increases. Further opportunities may be then explored.
