Moving on from care

If you’re aged 16–21 (16–24 if you are in higher education) and  leaving care, the Pathway Planning team can provide help and support.

We have a team of social workers and personal advisors who will talk to you about your plans for the future – things like your goals, interests, and where you plan to live. We can advise you on accommodation options.

We have two Connexions personal advisers attached to the team who can provide information about education, training or employment. We also have a nurse available to help you with your health needs.

We will work with you to bring all this information together into something called a Pathway Plan – which describes your planned ‘path’ into adulthood.

We welcome feedback about how we deliver our service, and have our own young people’s participation group. We also hold an annual event to celebrate achievements of young people.
