The fifth edition of the Leeds Long Term Care Charter (2004/2005) is now available.  Leeds Long Term Care Charters have been produced annually since 2000 by a partnership between service users, carers and the main organisations that plan, fund and provide community care services.   

Leeds Long Term Care Charter lets people know about the standards that they can expect from community care services and how to access these services.  The organisations that provide these services include the Social Services Department, local health organisations, the Neighbourhoods and Housing Department, voluntary and independent organisations.

We hope that the Charter will provide you with the information you need about the standards we are working to and how to access services.
