Application for a premises licence |
Use this form to apply for a brand new premises licence to authorise one or more licensable activities at a given premise. The licensable activities authorised by a premises licence are: the sale or supply of alcohol; the provision of regulated entertainment and the provision of late night refreshment between the hours of 23:00 and 05:00

DPS Consent Form
|
Use this form if you are making an application for a premises licence which includes the sale or supply of alcohol. This form must be filled in and signed by your 'Designated Premises Supervisor'. Used in conjunction with a PREM1. |
| PREM3 |
Application to vary a premises licence to specify DPS |
Use this form to alert the Licensing Authority that you are changing the named 'Designated Premises Supervisor' of a premises licence. |
| PREM4 |
Application to transfer a premises licence |
Use this form to transfer a somebody else's premises licence into your name. Should be used in conjunction with PREM5. |
| PREM5 |
Consent of premises licence holder to transfer |
This form is used by a premises licence holder to give their consent to their premises licence being transferred into another persons or organisations name. Used in conjunction with PREM4. |
| PREM6 |
Application for a provisional statement |
Use this form to apply for a provisional premises licence. This form is intended for premises which are being built or are about to be altered and or extended to allow licensable activities in the future. |
| PREM7 |
Application to vary a premises licence |
Use this form to apply for a variation of an existing premises licence. Eg to add extra hours or ask for licensable activities that the current premises licence does not allow. |
| PREM8 |
Interim Authority Notice |
This form is used to allow a premises licence which has lapsed (because for example its holder has died) to be reinstated to allow the business to continue trading. |
| PREM9 |
Notification of an interest in premises under Section178 |
If you are a freeholder or leaseholder who holds an interest in a property, which is the subject of a premises licence, you can use this form to register a notification of interest in the premises so you will be kept informed when changes take place to the premises licence. |
| PERS1 |
Application for a personal licence |
This form is used by an individual to apply for a new personal licence which allows the holder to authorise the sale or supply of alcohol. |
| PERS2 |
Disclosure of convictions and declaration |
This form must be submitted in addition to a PERS1, PERS4 or PERS3 and allows an applicant for a personal licence to submit a disclosure and declaration of convictions. |
| PERS3 |
Application to renew a personal licence |
This form is used by the holder of a personal licence to apply to renew the licence. Personal licences once issued last for ten years. |
| CLUB1 |
Application for a club premises certificate |
Use this from to apply for a brand new club premises certificate to authorise one or more licensable activities at a registered club. |
| CLUB2 |
Declaration for a club premises certificate to be granted |
This form is used in conjunction with a CLUB1 and allows the applicant to make a declaration that the business or organisation in question is a qualifying club as defined under section 61 of the 2003 Licensing Act. |

Application to vary a club premises certificate |
Use this form to apply for a variation of an existing club premises certificate Eg to add extra hours or ask for licensable activities that the current club premises certificate does not allow. |
| NOTICE1 |
Premises licence notice of application |
This form is used to advertise a new premises licence application. The form once filled out must be displayed on or at the premises. |
| NOTICE2 |
Club Premises certificate notice of application |
This form is used to advertise a new club premises certificate application. The form once filled out must be displayed on or at the premises. |
| NOTICE3 |
Premises licence notice of application to vary |
This form is used to advertise an application to vary an existing premises licence. The form once filled out must be displayed on or at the premises. |

Club premises certificate notice of application to vary |
This form is used to advertise an application to vary an existing club premises certificate. The form once filled out must be displayed on or at the premises. |
| NOTICE5 |
Notice of application for a provisional statement |
