If we are holding information about you in order to provide you with services, you have a right to see that information under the Data Protection Act (this is called a 'subject access request').

To do this you need to look at the leaflet in the downloads section - called 'Your right to see information about you' and complete the tear-off form at the end of this.  This should then be sent us to meet your request.

Leeds City Council Social Care takes your personal information very seriously and it is held in confidence.  Where it may be necessary (and often essential) to share your information, we will do so lawfully.  An important code of practice in doing so is set out in the Leeds Inter-Agency Protocol for Sharing Information which is available as a PDF download or through the Leeds Information Sharing (NHS) website linked below.
